No order form is necessary. Simply write the item name on the "memo" line of your check. If ordering multiple items or paying by US Postal Money Order, list the items on a separate paper. Remember to include the shipping charge, if any applies. Clearly write your shipping address on a separate paper, along with a telephone number.
 
For shipment to addresses in North Carolina, remember to add sales tax. Take the total price of your items, multiply by 1.0675, then add shipping charges for your total.

We accept only checks drawn on US banks, or US Postal Money Orders.
